Aadhaar Numberআপনার ১২ ডিজিটের আধার নম্বর লিখুন (বাধ্যতামূলক)

Beneficiary Nameমাধ্যমিক Admit Card অনুযায়ী নাম লিখুন

Date of BirthDD/MM/YYYY ফরম্যাটে (১ এপ্রিল ২০২৬-এ বয়স ২১–৪০ হতে হবে)

Guardian Namesপিতার নাম, মাতার নাম এবং স্বামী/স্ত্রীর নাম (যদি প্রযোজ্য)

Mobile Numberব্যাংক অ্যাকাউন্টের সাথে লিঙ্কড মোবাইল নম্বর
Section B: Address / ঠিকানা

Permanent Addressগ্রাম/শহর, পোস্ট অফিস, পঞ্চায়েত/ওয়ার্ড, ব্লক, জেলা এবং পিন কোড লিখুন। বিধানসভার নামও লিখুন।
Section C: Education / শিক্ষা

Madhyamik Detailsরোল নম্বর, পাসের বছর, বোর্ডের নাম (WBBSE/CBSE/ICSE) এবং প্রাপ্ত নম্বর লিখুন। উচ্চতর শিক্ষা থাকলে সেটিও লিখুন।
Section D: Bank Details / ব্যাংক বিবরণ

Bank Accountব্যাংকের নাম, শাখার নাম, অ্যাকাউন্ট নম্বর এবং IFSC কোড লিখুন। অ্যাকাউন্ট অবশ্যই single (joint নয়) এবং Aadhaar-linked হতে হবে।
মনে রাখুন: ফর্মের "Duare Sarkar Registration No." ফিল্ডটি খালি রাখুন — এটি পূরণ করার দরকার নেই। এটি offline submission-এর জন্য নয়।
